Youth Forms

Below are all the Youth Group forms. Each form is in two formats, namely Adobe Acrobat (.pdf) and Microsoft Word (.doc). Using the Acrobat version assures identical layout on all computers and is preferable if you are printing the form and then filling it in (if you do not know how to use Acrobat Click Here). The Word file is meant for those who wish to type their answers directly into the application and then print the completed form. In all cases any signature lines on the form must be signed by hand, in ink (and by a parent or gaurdian as well for anyone under 18 years of age). We do not currently provide any facility for submitting forms online, so please print all forms and return to whoever asked for the form or to the Youth Pastor below:

Parental Permission Form

To be used for all events requiring parental permission. Please be sure to a legal parent or guardian signs this form and that the youth brings it with them to the event or they may not be allowed to participate.

Yearly Parental Permission Form

You can now fill out a blanket form to cover most events in the course of the year! No more need for permission forms for each event.

Missions Application Forms

There are three separate forms needed to apply for any of the Youth Missions Trips. All three are below:

1) Short-Term Missions Application

2) Student Reference Form - for Parents to complete

Reference - Parent : Word Form (.doc)

Reference - Parent : Acrobat Form (.pdf)

 

3) Student Reference Form - for Other References to complete
